Alert user when code host is down

Administrator requested to merge mv/codehost-down-error-message into main

This PR adds a check for potential outages on GitLab and GitHub. When there's an outage, it displays an alert in the Code Hosts connection page (see image attached).

Test plan

All current tests should pass. Manually tested by simulating simulating 500 errors and the response from the GitHub app Screen Shot 2022-03-25 at 6 41 44 PM .
