Db doc updates

Administrator requested to merge db-doc-updates into main

This PR attempts to pull in more info from a recent upgrade failure and provide some explanation to those users less familiar with psql and sql commands. This still feels like a draft, reviewers please suggest comments on areas to expand or retract information.

Main changes:

  • integrated ending bullets into resolution steps, breaking steps into a tldr section and a detailed/explanation steps section
  • included a little more information about non-frontend dbs, INDEX_CONCURRENTLY, how to deal with partially completed migrations, links to how to access database shells in various deployments
