doc: include a direct link to GitHub with the right scopes

As suggested by sourcegraph/accounts#3923, we can prefill the required scopes on GitHub, rather than requiring the user to select checkboxes. I've kept the existing instructions, but now made them the "GitHub Enterprise" instructions, since that's where they'll apply.

Tested that the link works.
