Add extension architecture docs

Questions:

  • What's a good brief, but accurate, definition for "extension host"?
  • extension-host.svg, which I exported from app.diagrams.net, triggers 69 stylelint errors. Should we add SVGs to .stylelintignore, individually ignore errors, or just settle for a PNG? It would be sad to lose links from the diagram, but I understand we deem that there are too many violations to ignore.
