Only show "Close changesets on code host?" checkbox when there are no open changesets

If you have a manual campaign without any changesets, the "Delete" and "Close" prompts will show the checkbox, even though there are no changesets:

When you have a campaign in which all changesets are closed or merged, the same prompt also shows up:

Also note: the wording says "Close all 1 changesets". "All 1" is not correct :)
