managed: use issue links instead of hubspot links for customer instances

So that we can see the relevant deployments without access to Hubspot. Uses @malomarrec 's experiment to have accounts as issues we can link to : https://github.com/sourcegraph/accounts

Where the account differs significantly from the instance name, I used the original instance request issue instead.

Sample:

These managed instances upgrades are scheduled for $ONE_WORKING_DAY_AFTER_RELEASE.

To perform these upgrades, follow the managed instances upgrade process. Make sure to upgrade internal instances before customer instances.
