Can't reset password: Forbidden - CSRF token invalid

Sourcegraph version: Sourcegraph.com Platform information: Ubuntu 16.04, Chrome 65.0.3325.181 (Official Build) (64-bit)

Steps to reproduce:

1. Open in web browser https://sourcegraph.com/password-reset
2. Enter your email
3. Click "Send reset password link"

Expected behavior:

The reset password link is sent to email.

Actual behavior:

Error message appeared: "Forbidden - CSRF token invalid". No email was sent.
