Add CHANGELOG to `@sourcegraph/code-host-integration` package

Description

It's hard to see the changes between new versions of this package. Is it possible to use semantic commits in a way that automate logging to a CHANGELOG file that gets packaged along with the assets in the @sourcegraph/code-host-integration npm package?
