Code Insights [Creation UI]: Redesign insight creation UI

This issue implies updating UI parts on the all-creation UI pages

  • Remove placeholders and put example (prev placeholder content) above the filed
  • Group page sections with new background-based (filled) field group
  • Update the Header component (add a link to the insight icon, change the sub-header layout)
  • Update copy

Follow-up items from https://github.com/sourcegraph/sourcegraph/issues/38836

  • the container of the whole form is wider than for other code insight types - it should be narrower
  • we should add 'type: X | change type' element to all remaining insight categories for consistency
  • change the helper text in the header, remove the helper text below the cards (design)
