insights: associate insight views with specific users or orgs

Insight views represent a visualization into an insight data series. Currently, insight views do not relate to users or org, or any other future team structure. To match feature parity with settings based insights, we need to be able to provide a similar experience to a "settings cascade": global -> orgs -> user

Long term we can expect to support something like permissions (read / write, etc) on specific permissions. We should design with that in mind, but for now, we can work on the same assumption that extension-based insights use - if you can see the insight you can edit or delete the insight.

As an additional note, we will need to include this information in the settings migration jobs.
