Code insights filter and 3-dot menu button have no ARIA labels

The "filter" button and the 3-dot menu button are simply announced as "button" with no indication for what they do. This is really bad – they could potentially be destructive, so a user needs to get an indication what they do, and given there is no text label and only an icon, they need an aria-label.
