Canceling OAuth connection with GitHub doesn't return the user to Sourcegraph correctly

Steps to reproduce:

1. Start creating a code host connection with GitHub using OAuth
2. Hit "Cancel"

Expected behavior:

User returned to Sourcegraph on the code host connections view.

Actual behavior:

User is presented with a blank screen with the reponse:

oauth2: Request missing code or state
