Add handbook documentation for adding unplanned items to an iteration

We would like to make sure we can plan our iterations accurately and in many cases we get unplanned worked which we did not account for during our planning session.

Adding an item to our iterations means implicitly an item is being deprioritized and will likely fall out, we want to make this explicit in our handbook.
