RFC 201: Detect and report misconfigured index jobs

If index jobs fail consistently, it should be flagged as erroneous. This will help us detect things that aren't inferred correctly (this could help us with CNCF repo set being kept healthy, for example), or have drifted from a previous configuration that no longer applies.
