RFC 201: Create index record based on database configuration

This is part of RFC 201: Auto-indexer build configuration.

Add database tables to configure indexing for a repository. If a repo has a configuration in the database, this should be used instead of the canned configuration published for any repository. This may result in many index records per repository as this will respect roots, of which multiple can be defined.
