RFC 201: Create index record based on configuration file

This is part of RFC 201: Auto-indexer build configuration.

If a repo contains an index configuration file, this should be read and used instead of the canned configuration published for any repository. This may result in many index records per repository as this will respect roots, of which multiple can be defined.
