Add an extension UX guide

This can act sort of like a coding style guide, but for extension UX. When there's contention over some extension UX, this doc can be used as a basis for supporting one behavior over another.

Does anyone disagree with these recommendations? Any new ones to add? This guide is not set in stone and will probably get amended over time.
